I am aware that I can revoke my consent at any time.__________________________________ WAIVER: In consideration of all players listed on this roster being permitted to participate in any way in the QUICK 6 FOOTBALL's 2023 GOLDEN GREAT 7-on-7 CHAMPIONSHIP Tournaments (both high school & youth divisions), I, for myself, all players listed, all parents of listed players, my heirs, personal representatives or assigns, do hereby release, waive liability, assume the risk and covenant not to sue any QUICK 6 FOOTBALL representatives, Premier Concepts Marketing and Promotions, participating Event Partners, Event Sponsors, volunteer coaches, directors and officers for any personal loss or injury (Including Death), accidents or Illness, and/or property loss resulting from any of these players' participation in the aforementioned 2023 GOLDEN GREAT 7-on-7 CHAMPIONSHIP Tournaments, including, but not limited to anyone’s negligence and/or improper conduct. I hereby assert that all listed players' participation is parent-approved and is voluntary. As head coach or lead representative of the participating players listed, I knowingly assume all such risks. Photo & Video Waiver: All images of participating player(s), captured during the 2023 QUICK 6 FOOTBALL 7-on-7 football games through video, photo and digital phone camera, will be used solely for the purposes of QUICK 6 FOOTBALL promotional material and publications, and therefore, the player, coach & parent waive any rights of compensation or ownership thereto.
