I hereby assert that all listed players' participation is parent-approved and is voluntary. As head coach or lead representative of the participating players listed, I knowingly assume all such risks. Photo & Video Waiver: All images of participating player(s), captured during the 2023 QUICK 6 FOOTBALL 7-on-7 football games through video, photo and digital phone camera, will be used solely for the purposes of QUICK 6 FOOTBALL promotional material and publications, and therefore, the player, coach & parent waive any rights of compensation or ownership thereto.
