COACHES:
If you are a coach with an established team, please submit your team's roster(s) with
the name of each player. The TEAM REGISTRATION FORM is located below on this page. Also, each player must have a waiver form filled out by his parent or legal guardian to participate. This form
must be presented prior to any player being allowed to participate in any QUICK 6 on-field activities.The QUICK 6 PLAYER REGISTRATION AND WAIVER FORM is also located on this
page.
TEAM ENTRY FEES for the QUICK 6 JR. PASSING LEAGUE (2014): All team entry fees are based upon the # of players that a coach will have on his/her team(s). There are a minimum of 7
players and a maximum of 21 players allowed on each team. A head coach is allowed to have a maximum of two teams per division. The team entry fee is $75.00 per player (example: if you have 10 players
that you are registering on a team, the team entry fee is $750.00). Parents who choose to pay for their child's registration fee may do so individually online, Just follow the simple instructions on
this page (See PLAYER REGISTRATION FEE section)

PARTICIPATION WAIVER on this page, print it and fill it out. Your child will not be able to participate in any of our events unless we have this form in our database. Please, no nickmames
when filling out this form.

Hello Coaches & Parents:
It is in this section that you can make your player(s) entry fee for the 2014 QUICK 6 JUNIOR 7-on-7 PASSING LEAGUE. We suggset to keep things in order that a team
register collectively as opposed to each parent registering their child individually. However, we are set up to process either way so please read the following instructions:
COACHES: If you are going to pay for your team as a whole, please make sure that you parents are aware of your intentions. That way we do not
receive double payments for an individual player. When paying the team registration fee with PAYPAL, please enter the amount of players you have on each squad (ex: 10 players x $80.00 = $800.00). If
you have any questions registering on making the payment, please contact Rob Sutton at (916) 222-8750 for assistance.
PARENTS: If you are registering you child as an individual through PAYPAL, please make sure you print out the receipt as proof of purchase (note: There is a $5.00
service fee for online payments). Bring you receipt along with the filled out PLAYER REGISTRATION FORM when checking in on June 1st, 2014. If you have any questions registering on making the payment, please contact Rob Sutton at (916) 222-8750 for
